What is Coordinated Family Support?
Coordinated Family Support (CFS) is a service for adults who want to live with their family but need help coordinating support, learning skills, or planning for the future.
Coordinated Family Support was created by the Department of Developmental Services in response to input from families about the lack of available services for adults who want to live in the family home.
Coordinated Family Support services must be:
Provided in the same language as the client/family
Respectful of the individual and family culture
Tailored and flexible to meet the unique needs of each individual and their household.
What is Teaming Up?
Teaming Up provides individualized support for adults living with their families through education and training, person-centered planning, and collaboration with other supports. A service plan is designed to help reduce or remove barriers to staying in the family home, address challenges and meet goals. Services are tailored and flexible to meet the unique needs of each individual and their household.
Teaming Up Coordinated Family Support Services include (but are not limited to): future and goal planning, safety and emergency planning, establishing a network of service providers and backup providers, identifying transportation resources, providing education and training to maximize self-advocacy, independence and consistent care, and assisting with scheduling of service delivery including medical and other appointments.
